The plot follows two American tourists, Lindsay and Jenny, whose car breaks down in a remote German forest. Seeking help, they arrive at the villa of Dr. Heiter, a world-renowned expert in separating Siamese twins. However, Heiter has grown obsessed with a "reverse" medical experiment: instead of separating bodies, he wants to join them together.
